You must set up an appointment to get checked-in and cleared by Sports Medicine before beginning athletic activities: SIGN UP HERE 

Please fill out all forms completely. All forms need to be returned to: sportsmedicine@citadel.edu


Before downloading any forms:

• Please make sure the latest version of Adobe Acrobat is installed on your computer.
• Be sure to download the PDF file to your computer in order to complete the forms. Open the form in Adobe, click the arrow on the right to open the tool bar and click "Fill and Sign" on the window on the right side of the page, and begin. Make sure you save the new document with your first and last name at the beginning.
• If printing the form out to fill out by hand, please use blue or black ink only and print clearly. If you wish to fill out via Adobe Acrobat,
• An Alternate Option: on your phone/tablet, download the app "Camscanner." This app allows you to take a picture of a form and it will automatically convert it to a PDF file.

For New Student-Athletes:
Dear Parents and Student-Athletes:
The following forms will need to be filled out completely and returned to The Citadel  Sports Medicine Department via email to sportsmedicine@citadel.edu . Proof of Sickle Cell Trait Testing:
The NCAA requires all student-athletes to be tested for sickle cell trait. Please enclose upload proof of your sickle cell trait results. You will not be allowed to participate unless you provide proof of testing with results.

For Transfer Student-Athletes:
All medical records from your previous school must be sent to Sports Medicine to include sickle cell trait results. Fax: 843-953-1465  Email: sportsmedicine@citadel.edu
